Permission Slip (copy quickly)
We will be watching some videos with mature content. In order to move forward on this, I need signed permission slips. Copy the following and take it home tonight. ALL permission slips are due tomorrow, or I will need to alter the course to remove the films. Ask your parents to read it and sign it if they agree:
I give my student _ permission to view films in English 4C that have mature content. I understand that the course will deal with science fiction and victimization and it will contain films that accompany those fields of study. I understand that some material is rated PG-13 by the Motion Picture Association of America for violence, brief language, some sexuality and drug content and some material is rated R for violence and brief strong language. I also understand that my child will view a video showing graphic images taken during the Jewish Holocaust. I understand that the teacher chose these films because of their relevance to the course material and I give my student permission to view the films in class. I also understand that if I do not agree to allow my child to view the films that he/she will be given an alternative assignment.
I understand the above paragraph and I give my student, _, permission to view the films in question.
